Implants

Every dazzling smile has its strong basis. Regardless the origin of its foundation our tooth is always subjected to high stresses. The stresses are usually extemely large going well above 60kg per tooth. That is why a dental defect cannot be replaced by a badly designed system. Luckily, in the 50s and the 60s Ingvar Branemark and his co-workers have discovered a mechanism to join titanum elements with a surrounding bone. The event has been a great milestone in the history of dental implants. People have realized that a biologically active and pollution free implant is indifferent to the bone. Following the discovery, they kept on developing the perfect implant system.

What is a dental implant?

A dental implant is a small 7-21 mm long nut whose diameter is usually between 3-4 mm. It is the component to replace the natural root of your tooth. No wonder it looks like it. It is made of pure titanum - material indifferent to the tissues of our body.

Recommendations

We recommend implants in the following circumstances: loss of one tooth, loss of a few teeth, or toothlessness.
The tooth loss can be due to an accident, late treatment of a sick tooth, parodontosis. In addition to that, a patient may not be able to develop dental compounds on his/her own. All of the above make implants inevitable.

• Loss of one tooth - if only a single tooth is missing, implants are a pefect solution to your problem. It develops a steady foundation for the crown, and first of all, it does not require filing the two neighbouring teeths for the sole purpose of placing a dental bridge on them.
• Loss of a few teeth - the best solution is a bridge assembled on a multiple implant system. Such assembly can be applied regardless the shape of the neighbouring teeth. The bridge is set on implants operating on its own without the need to file any teeth.
• Toothlessness - we recommend a prosthesis assembled on an implant system. The following solutions are available here:
  • Prosthesis assembled on implants works best with patients having large and strong bones. The procedure requires setting up between 6 to 10 implants in a jaw-bone. Next, a stomatologist assembles a permenent prosthesis on the implant system.
  • Prosthesis partially assembled on implants - can be assembled on two regular implants. However, four implants in a trapezoidal configuration ensure better stabilisation. In either scenario we apply split via a cantilever.

Contraindications

Implants requires a thorough medical examination by a stomatologist during a pre-visit. Then, based on the RTG/RVG scan (which we carry out on-site), our expert selects the best possible implant system or advises against it if any contraindications occur.

Implant systems

Selection of a proper dental implant system is decided on per-individual basis. A stomatologist selects an implant based on the following criteria: the amount of bone in a the planned spot, the jaw-bone section as well as the overall bone shape where the implant is to be planted. In our clinic we use only the best quality implant systems from Nobel BioCare developed in research institutes and supervised by the best quality specialists. The implant are covered by a 10-year warranty.
We usually recommend the folowing implant systems: Nobel Active and Nobel Replace Select Tapered.

• Nobel Active Implant, due to a purpose-profiled section of the implant makes the bone density increase locally in the neighbourhood of the tooth. The phenomenon accelerates faster bone-to-implant integration as well as implant stabilisation, too. The implant system features an aesthetic prosthetic joint.
• Nobel Replace Select Tapered Implant is a cone-shaped implant that mimicks the natural shape of your tooth root. The implant can be used in cases where there is a shortage of space (between tooth roots). We recommend them in cases that require an immediate implant treatment following a tooth extraction.

Treatment process

Implant system classification:

• Conventional implantology - as soon as an implant is in place, the gum gets stitched to cover it. 7 to 10 days after the sewing a stomatologist removes the stitches, and after a period of 5 months it applies a healing bolt on an implant. At this stage we sew the gum once again. The purpose of this pre-treatment is to "form" a gum for the future prosthetic work. Approximately, 10 days following the treatment we collect a preliminary impression to form a prosthesis. This is the end of the surgical part of the treatment
  
• Immediate implantology - the implant treatment follows tooth extraction, and an implant is introduced to replace the removed tooth. All of it takes place during one visit. As the natural tooth root is wider than an implant, quite often the remaining space is filled with a special Bio Oss bio-material formed out of beef bones.
• Immediate (loading) implantology - research have proved no contraindications on loading implants as soon as they are set in place. On the contrary, scientists claim that a pressure on an implant stimulates and prompts the surrounding bone to integrate it with the implant. For patients who require crowns applied on implants almost immediately we offer the following solution: using special implants we can load them with a crown or a bridge following the implant treatment. We assemble a temporary bridge system or a crown, and our patient leaving our clinic can function as ever before.
  Combining the above treatments is ideal. Our patient saves his time, and is encouraged to return to his regular lifestyle as soon as possible.

Bone augmentation

In some circumstances we (stomatologists) come across a patient not having enough bones to carry out the implant system. In the past, the only solution was to remove a piece of a bone from the patient's hip and use it in the planned place. Such treatment was a complex one, to say the least, and not without complications. Stomatologia Cichoń have successfully used a false bone system from Geistlich- Bio-Oss, Switzerland. The material quickly develops solid basis for an implant, and is used in maxillary treatment as well.

The whole procedure is based on a computer tomography scan of a jaw. The scan forms basis of a 3-dimensional model of bones and enables us to plan the implant procedure at every smallest detail, forming the prosthesis even before the patient's arrival at our clinic. Prosthetics

Prosthetics, by origin, has been a a treatment to restore a patient's chewing organ functionality. Its functionality, however, has evolved over the years - nowadays prosthetics works towards achieving ideal easthetic effects.
Prosthetics is an active and resilient branch of stomatology.

Treatment process

We offer the following solutions:

• Loss of one teeth, a few teeth, toothlessness - every prosthetic solution is selected specifically for an individual patient. With patients who have experienced the loss of one tooth or a few teeth we use crowns and bridges. First, our expert sets an implant in the right place, then assembles a crown on it. An alternative is possible, too - a 3-point bridge (3 inter-connected crowns) assembled on the neighbouring teeth.
  Crowns and bridges are assembled on a metallic base (steel, gold, titanum), or a non-metallic ceramic one. The right treatment depends on the aesthetic effect that is desired by a patient. In front sections we use ceramic crowns - they best reflect the natural color of a tooth. The ceramic crowns are made in the PROCERA system.
  When using conventional crowns one biggest disadvantage is a gray line that develops between a crown and a gum.
  First, our stomatologist – prosthetician, prepares a tooth for a crown, then takes dental imprints and sends them to our prosthetic laboratory J. W. Labor. The PROCERA system is manufactured in Sweden using a zirconium base to replace a conventional metallic one. Our Polish laboratory develops a virtual model of the tooth which they send to Sweden where the crown is machined and computer-milled. Once complated, the crown gets shipped back to Poland where a technician covers it with ceramic layers.
  Most of the manufacturing process is computer-based effectively replacing a technician (a human being)

• Procera Implant Bridge is a system designed specifically for implant-based prosthetics. It does not require applying additional components between implants and a crown which makes it a very tight solution at an affordable cost. Dental bridges manufactured in this system are made out of one piece of a bio-material - titanum or zirconium. They are prepared in Sweeden using state-of-the-art scanning and computer-based milling. Moreover, Procera Implant Bridge can be used for patients with toothlessness. In our clinic we use the following solutions:

• Loss of a few teeth - we offer an extremely stable telescopic prosthesis. They provide a very stable support to loose teeth, and we recommend them for patients with parodontosis.
• Loss of all teeth (toothlessness) - Patients without any teeth usually choose a prosthesis that is assembled on implants. A conventional gum-mounted prosthesis is not popular anymore. It does not meet its fundamental functionality - it does not restore 100% chewing capability in a patient. Moreover, forces that are transmitted via acrylic and gum to the jaw-bone cause it to decline gradually in time. The decline develops a loose space between a gum and a prosthesis, thus loosing its fit quality. Patients who in the past used the prostheses complain about them as very uncomfortable in their everyday life. They claim eating is easier for them without a prosthesis! In this case, one ideal solution is a prosthesis assembled on implants. Toothlessness solutions:
  
  
  • Procera Implant Bridge
  • 2 or 4 implants (depending on the jaw-bone shape) and assembling of a permanent prosthetis or a highly aesthetic telescopic prosthesis.
  • 4 or 6 implants and a permanent crown-bridge system. That is the most aesthetic and comfortable solution developed so far.

Veneers

A veneer is a plastic, porcelain or composite material used to improve the attractiveness of a stained, coloured or mechanically damaged tooth.

We use the following veneer types:

In case of patients with no tooth crown sections we use prosthetic crowns.

Whitening

Whitening is a treatment used by patients who accept the shape of their teeth, and yet they desire to achive the dazzling snow-white smile. In STOMATOLOGIA CICHOŃ we use the most recent and safest whitening system from BEYOND. Whitening is absolutely painless, and it takes only one 1h visit. Once the procedure is finished we ask our patients not to drink nor eat anything that might colour their teeth (coke, coffee, ...). The whitening procedure is comfortable and very effective - 8 degrees on the VITA scale.

Endodontics - root canal treatment

Endodontics is is a specialist sub-field of dentistry that deals with the tooth pulp and the tissues surrounding the root of a tooth. In our clinic the root canal treatment is carried out by an expert stomatologist - trained endodontist who specializes in the particular field. During treatment, our endodontist controls the treatment progress on a RGV camera from Gendex in real-time. It allows for a perfect filling of root canals, and prevents any problems or infections in the future. In difficult cases we carry our the root canal treatment using our Leica microscope. The apparatus allows us to pin-point other problematic root canals, restore narrow canals or remove broken endodontic tools from the root canals.

In Stomatologia Cichoń root canals are not treated conventionally. For the root canal treatment we use the state-of-the-art equipment from Obtura. It allows us to fill root canals in a precise manner to guarantee no side effects or problems to appear in the near/long term future.

Ozone therapy

HealOzone is a gentle yet extremely efficient way of treating your tooth decay. The system has been designed for children and adults having unpleasant memories of dental treatments. It is one of the most recent achievements in the modern dentistry. As one of the first dental clinics in Poland we have introduced the therapy into our procedures. The therapy is painless. The idea of the intervention is to inject ozone through a silicon cap that covers your tooth. At first, the gas kills all bacteria that contribute to the disease. Then, it hardens the sick tissue of your tooth.

What causes pain during local anaesthetic?

Conventional injections make you always experience pain (a sting). Reserchers have proven on numerous occasions that the unpleasant sting using a traditional syringe is caused by firing the aneasthetic too quickly. The Wand™ is 100% painless and safe to our patients.

What is The Wand™?

The Wand™ is a computer controlled dental injection system where the flow rate of the local anaesthetic is controlled by a computer.

Why is The Wand™ injection painless to our patients?

The Wand transfers the anaesthetic fluid at a very slow rate, and that is all controlled by a micro-computer. With The Wand the fluid gets absorbed by your tissue not pressurized into it as with conventional syringes. The computer transfers that fluid at a precise flow rate that is imposible to achieve in a comventional way. Prior to the procedure the anaesthetic liquid gets sprayed on to your gum. Next, the computer starts applying the anaesthetics at every syringe's movement into your tissue to anesthetize it.

Orthodontics

Modern orthodontics provides us with powerful tools to treat your malocclusion. In our work we use resident as well as movable orthodontic devices.

1. Movable - they are usualy applied by a patient and used in his/her sleep. They are used mainly with kids and youngsters. However, they lack the ability to determine the real effect of the apparatus.
2. Resident - fixed (glued) to the surface of your teeth. They usually com in 2 flavours: metallic - standard, cosmetic - coloured to match the colour of your teeth. The former is used only by few dental clinics in Poland, and is fixed to your teeth on the inside. We recommend it to patients whose profession and/or lifestyle do not permit standard procedures.

Convervative treatment

Prophylactics is the key element of our treatment of children and adults.

One of the most important factors in fighting dental deposits (tartar) is the hygien of your oral cavity (mouth). Tooth brushing or dental threads are not easy to use properly! Choose your brush or threads with care for best effects.

Best efects can be easily achieved using fluoride-based fluids (0,02-0,05% of fluoride: 6-year old kids or younger, 0.1% or more: adults). They should be used twice a day upon brushing your teeth. Everyday prophylactics includes fluoride-based toothpastes as well. However, pay attention to the fluoride content in the toothpaste: it varies with age and dental indispositions (tooth decay).

Children

Our desire is to make sure your child enjoys a healthy smile for years. That is why we pay special attention to your child, and our staff is trained on children's stomatology.

We offer you FREE adaptation visits, during which our expert will spend his/her time on chatting with your child, etc. for minimum stress and to avoid any unpleasant experience. Therefore, as one of the first dental clinics in Poland we have introduced the ozone therapy for no-drilling no-pain experience. When it comes to a child the very first visit at a dental clinic is the most important thing in the world. There can be no pain nor fear.

Our offer for the little patients:

• Coating - using brushes we coat teeth with a special fluoride varnish for protection against always-dangerous tooth decay.
• Tooth fissure sealing - the surface of wall teeth contains spots liable to tooth decay. To prevent that, we cover them with a special protective liquor.
• Ozone therapy - no drilling, no pain, no tissue loss.
• Air-based treatment - for big "holes", or old filling remowal we do not use any conventional techniques (drilling). We use abrasive air jets. High-pressurized air mixed with tiny pieces of sand removes the sick tissue slowly without any unpleasant experiences.
• Coloured fillings - we use them with primary teeth. Kids like them, and they are easier for parents to keep an eye on and react accordingly.

Tooth decay diagnostics with the KAVO DIAGNODENT laser

The laser allows us to detect the smallest tooth decay that is invisible to the human eye. As such, we can fight any tooth decay even before the unpleasant "hole" shows. The tooth decay treatment is absolutely painless, and the early discovery of the problem allows us to apply non-invasive techniques using Kavo HealOzone.

AcuCam® Concept IV FWT Intraoral camera

A thorough check of your tooth cannot be reliably examined using classic methods - the mirror. That is why we use digital cameras by Gendex. A stomatologist introduces the camera into your mouth. The picture is transmitted to a computer and displayed on a monitor in real-time. The feature allows us to perform a thorough check of your tooth, and for patients to have a look at their own tooth before and after the treatment.

RTG

For X-ray type photography we have installed RTG digital cameras. They meet the highest safety standards - they do not generate any auxiliary radiation, and the X-ray dose a patient is exposed to is minimized to the absolute minimum. Using digital cameras we can analyze the picture on a computer as soon as it is taken, getting much more information than from a conventional X-ray photography. A patient receives a copy of the photo on a CD or a printout.
